The Plan

  • 2 morning measurements.

  • ​2 night measurements.

​

​* For a period of 7 days, before/after your next appointment.

​

*If medications are renewed follow the same plan, but for 2-3 days.

  • Systolic/top reading for the two trials.​

​

  • Diastolic/bottom reading for the two trials.​

​

​* Any additional comments for your doctor.

​

*The date and time when the readings were conducted.

​

*Everything can be recorded in the sheet below.

Recommended steps (Optional)

  • No smoking or drinking caffeine 30 minutes before measurements.

  • Sit quietly with limited movements while the reading is being taken,​

​

*Benefits of tracking your blood pressure; helps us determine if your blood pressure is controlled and it shows your progress towards improvement.​
